Everything You Need to Know About Baby Eczema Patches, Hot Spots, and Dry Spots

Baby skin is sensitive by design, which means dryness, rough patches, and irritated “hot spots” are incredibly common. As pediatricians, we see these every single day, and the good news is: most cases are manageable at home with the right routine.

Here’s a quick guide to understanding the most common types of baby skin irritation - and how to soothe them gently and safely.

What Are Eczema Patches?

Eczema (atopic dermatitis) causes dry, red, itchy areas often on cheeks, arms, legs, or behind the knees.
These patches come and go, flare with triggers, and tend to improve with consistent moisturizing.

What helps:

  • Daily fragrance-free moisturizer

  • Short, lukewarm baths

  • Avoiding fragrances, dyes, and wool fabrics

  • Using a barrier balm on dry or inflamed spots

If patches are very red, weepy, or spreading, check with your pediatrician.

What Are “Hot Spots”?

Hot spots are areas where the skin becomes inflamed and irritated due to heat, sweat, drool, or friction.

Common locations:

  • Neck folds

  • Underarms

  • Behind the knees

  • Diaper area

What helps:

  • Keep the area clean and dry

  • Apply a thin layer of a gentle barrier balm

  • Dress baby in breathable cotton

  • Avoid overdressing (easy trigger!)

What About Simple Dry Spots?

Nearly every baby gets dry patches — especially during winter, after baths, or during growth spurts.

What helps:

  • Moisturize at least once daily

  • Apply balm immediately after bath time (“soak and seal”)

  • Use a gentle wash only a few times per week

  • Avoid scented products

Dry spots often improve within days when the skin barrier is supported.

When Should You Call the Pediatrician?

Reach out if you notice:

  • Oozing, yellow crusting, or signs of infection

  • Severe itching disrupting sleep

  • Rash in the diaper area that doesn’t improve in 2–3 days

  • Widespread or rapidly worsening redness

You’re never overreacting — baby skin deserves attention and care.
